To apply for a Mobility Travel pass because you have a disability or medical issue(s) that affects your mobility.

What we need from you
We will ask you to register for a MyDoncaster account if you haven’t already. This will allow you to track your enquiry, and for us to tell you the outcome of your request. You will also be able to access additional online services offered by us.
We will ask you to tell us details about the disability or medical issue(s) that affect your mobility.
You will also be asked to supply evidence to support your request. This can be uploaded as part of this form, or supplied later. If you do not supply the required evidence, this may cause a delay to your application being processed.
What you should expect from us
Once submitted, we will decide if you are eligible for a Mobility Travel Pass within 7 days. If successful, we will send you an entitlement letter so you can obtain your pass from SYPTE. We may need to contact you if further information is required to process your request.
