Together Information Exchange (TIE) Application form
What is this for?
For parents/carers of a child with a disability to apply for the TIE activity discount card and become registered on the TIE database.
Together Information Exchange (TIE) is Doncaster's voluntary register of children and young people with a disability. The register is administrated by Families Information Service and contains confidential information on children and young people with a disability.
What is it going to cost?
The card and application is free. Details of costs for individual activities appear on the TIE website, however, many are free or discounted for TIE card holders.
What we need from you
Please complete the form as fully as possible, including a telephone number and email, so that we can let you have up to date information. We also require evidence of disability – further details are on the application form.
Please provide 1 passport style photograph of the child to be used on the TIE card.
What you should expect from us
We will automatically acknowledge receipt of your electronic form and photographs and aim to get your new card out to you in 6 weeks.
