We will listen to you to understand:

  • what is important to you

This conversation is sometimes called a needs assessment.

We will make sure that you are fully involved in any conversations and decisions about your life and your care and support.

We will also listen to the views of other people who are important to you.

You might like to have a family member or friend with you to help you take part in conversations and decisions.

This type of support is called advocacy.

If you don’t have anyone else who can help you, you have a right to an independent advocate.

When we have listened to you, we will work out if you have eligible needs for care and support.

If you have eligible needs, this means we must make sure your needs are met.

If you don’t have eligible needs, we will still make sure you can get the right support.

How we check eligibility

If you have eligible needs, we will work with you to agree the support you require to live a good life and to do the things that matter most to you.

This is called care and support planning.

We will make sure you have a care and support plan, and that your plan describes the support that will work for you.

We will never ask you to pay more than your financial assessment shows you can afford.

We’ll contact you again once a year to see how you are and if your plan is still working.

This is called a review.
