The transition between settings can be particularly difficult for children and young people with special educational needs and disabilities, but being informed and prepared for the transition process can help to support children, young people and their families and to ensure your statutory duties are met.

What does the SEND Code of Practice tell us?

6.57 SEN support should include planning and preparation for the transitions between phases of education and preparation for adult life (see Chapter 8, Preparing for adulthood from the earliest years). To support transition, the school should share information with the school, college or other setting the child or young person is moving to. Schools should agree with parents and pupils the information to be shared as part of this planning process. Where a pupil is remaining at the school for post-16 provision, this planning and preparation should include consideration of how to provide a high quality study programme, as set out in paragraph 8.32.

inourplace | Solihull Approach – Moving up

Primary to Secondary Transition

We are part of an innovative pilot project with The University of Manchester. A number of schools across Doncaster are participating which benefits year 5 and year 6 children. Learning from this pilot will be disseminated across Doncaster in the coming future.

As a local authority we aim to identify all children that may struggle to make the transition for Primary to Secondary education. We have introduced a screening tool based of the research from University College London. This will help us to identify, support and intervene with this cohort of vulnerable children. We will also be able to track their progress through secondary education to measure the impact of our transition support.
