The transition between settings can be particularly difficult for children and young people with special educational needs and disabilities, but being informed and prepared for the transition process can help to support children, young people and their families and to ensure your statutory duties are met.

Primary to Secondary Transition
We are part of an innovative pilot project with The University of Manchester. A number of schools across Doncaster are participating which benefits year 5 and year 6 children. Learning from this pilot will be disseminated across Doncaster in the coming future.
As a local authority we aim to identify all children that may struggle to make the transition for Primary to Secondary education. We have introduced a screening tool based of the research from University College London. This will help us to identify, support and intervene with this cohort of vulnerable children. We will also be able to track their progress through secondary education to measure the impact of our transition support.
